Please bring the following items to your first visit:

  • Completed Registration Forms
  • Valid driver’s license, passport, or equivalent
  • Health insurance cards
  • An insurance authorization or a written referral from your primary care doctor, if required
  • List of current medications
  • Diagnostic imaging (e.g. MRI, CT scans) and reports on CD; EMG / nerve conduction studies; any other testing or prior records relevant to your current condition

Late Policy

Our team does their best to see you in a timely manner. Unfortunately, a patient arriving late often impacts the surgeon’s schedule for the rest of the day. Out of respect for your fellow patients, we ask that you please arrive 10 minutes prior to your scheduled appointment. We make all attempts to accommodate patients who arrive late, but may need to reschedule a late patient’s appointment so as to not compromise on providing quality and timely care.

As neurosurgeons, surgical emergencies do occur, which may result in delays or rescheduling appointments. While rare, we apologize in advance for any inconvenience this may cause you and will make arrangements to ensure you are seen as soon as possible.
